Liability Form for the City of Orillia's Recreational Facilities. 

Completion is required each booking. This form is to be filled out by a parent or gaurdian, with the participant, for facility users UNDER the age of 18.  

Ontario requires proof of vaccination to access any City of Orillia Recreational Facility. Please bring your proof of vaccination and a personal identitification card to each booking.

Please have the confirmation page ready for when you enter the facility to show the screening staff. You must complete this form daily.
